To save you time and effort during your visit, and to help make your visit go even smoother and faster, we’ve pre-loaded our Well Child Check-Up Questionnaires, as well as many of our most important screening tools, here on our web site. The forms are secure, just print and complete them to bring with you to your visit.
Filling out a form, before your visit, means we have all the information we need — including any questions or concerns that you’d like to talk about with your provider. And it also means you save time in the office!
Just fill out the appropriate questionnaire a few days before your next visit, and we’ll be ready to go when we see you in the office, and in the exam room!
